We tested in different environment and different facilities and part was behaving same with always. I suspect there is something on board which got corrected itself while I was probing the board.
My output load was a low pass filter with 1k resistor and 1nF capacitor. It goes then to a ADC input which may draw 1uA.
I did not measure the quiescent current. Issue is not there so I am first trying to replicate it again.

Actually we did enough validation to prove the op-amp operating limits. Base on this we used to write a new spec. Only thing surprising me is, why one board out of thousand boards shows this issue and it disappeared and now my board works good.

2. Are they electrically insulated?
3. Is there a particular length defined while detecting a break (length of cable from the twister)

I am using an Atmal micro controller for my application and it looks like it needs at least 15mA current during startup.
If it does not get supplied this current, it VCC gets latched to 1.45V. When it gets 15mA supplied for atleast 5msec then it came up and let the VCC go to regulator output...
you can model yourself. Use an inverter and RC at output and fed output back to input. By changing RC, you should get 60MHZ pulse (not square). use a flip flip based frequency divider and it will give you 30MHZ square wave at output.
you can put your resistor in series with LED and ground the source. It should help you.
The topology you are using may not let the FET turned ON fully and having a finite resistance.
