why high freq OP grow heater?

AD830, ±12V supply. input to 0V. after 1 minute, it become very heat, anyone can tell me why?

pls check for the pins and their connections... this will happen if there is a short circuit due to pcb or internal circuit.

dont connect the requlator to ckt .. test it seperatly with the data sheet spec
 

there are no any short, circuit function is good, but AD830 grow very heat.
