Does it simply mean that the output is of a square wave form, or rather has defined logic levels as opposed to an analogue waveform?

Thanks in advance

A gate in your room also allows one to pass if open, or one has to stop if the gate (i.e.door) is closed. Some operational amplifiers have "gate" input, by which the output is blocked . You can use such blocking gate to prevent a signal to get to output for a selected time.

A gate is basically a switch to turn the output on or off, as desired. It has nothing to do with the type or shape of the signal being gated. It can be either an analog or digital signal.

As discussed above, Gated output is just the controlled output with use of a gate(generally a transistor). Moreover generally these outputs are digital outputs.
