sigma delta unstable in simulink

sigma delta unstable

i used 2nd order sigma delta modulator model in simulink. its second integrator output is going way high and feed back is negative for sure, what would be the problem and suggest any parameters to be set for blocks in simulink. screen shot of the model is attached. Thanks in advance.
