RS485 fail-safe and TVS in the same implementation? [MAX481]

Hi! I have a pretty tight deadline, and am pretty confused with all the info I received from the manufacturers of the RS485 drivers (Application notes and datasheets).
My problem is: what should the termination i my case look like? I have multiple transceivers on the line (app 40m), and the drivers are all MAX 481 (half duplex). I know that the 120R (typical) resistors should be connected at the far ends of the cable, but what do I do for the fail-safe, and possible transient voltage surges (it is aerial cable)? Is the fail safe resistor network needed only once per cable, or not? Can I combine resistors for fail safe with capacitor in series with the 120R? How do TVS's for protection fits in?
