PWM Detector Using Integrator

How can I demodulate a PWM (Pulse Width Modulation) signal using an Integrator ?
 

You might use an even simpler, RC network. All you want is
the time-averaged voltage.
 

A LC low pass filter can also demodulate a PWM back to sin waveform. This is the principle used in class D amplifier.
 

An integrator can determine the duty cycle in exactly one pwm period, but it would need a sync signal with modulation schemes
that are shifting both edges. Furthermore it needs a sample and hold at the integrator output.
