Phase modulation using PLL

I am looking for a circuit which can phase modulate a carrier of 4000MHz.

Modulating frequency in the range 100Hz to 200KHz.

Can anybody give some clue or suggest a circuit to do it with PLL?

My requirement is to get 1.5radians phase shift at this frequency,
i.e the first sideband occuring at these 4000MHz+-Fmod frequenciies
should be about -4dBc or less.
