how to re-direct an IP?

Hi all,
I have a url.dyndns pointing to my PC's router dynanic IP.
I forward specific ports to an application running in one PC behind the router, which receives and sends using TCP/IP:

remote client uses url.dyndns ==> connects to router IP which forwards ports to app ==> app rcvs and xmts TCP/IP communications

Now I would like to move that application to an external PC at home:

remote client uses url.dyndns ==> connects to router IP which forwadrs all TCPIP traffic in specific ports to an external IP ==> app rcvs and xmts TCP/IP communications

Is this setup possible?
Is there any (free) application which I can use to do this?
