Max232 Hot-plug problem

Dear all,

I have an equipment with rs232 output (TTL level with 5V power source).
I made a adapter to connect that equipment with PC use max232 transceiver.
When the adapter is cold plug, the board work well.
But when hot plug, the transceiver IC became hot and can not work.

Could you suggest how to solve that problem?
If we use the connector with longer length, it will be OK?
Can we add some filter?
