just go thorough razavi.it is explained in detail how to assign bias currents and also over drive voltages for nmos and pmos transistors in the cascode branch.from which u can calculate the sizes.

Philip Allen's book gives a good description and design procedure, what is most important. It point out the problem you have to consider.
