"matrix analysis and applied linear algebra" from siam publications has some info on ill conditioned systems at the end of the first chapter. That book might have also extended this to matrices. It is available in edaboard as well.

there is also an example of such matrix in adaptive filtering books, in this respect you may recognize the concept of this issue better.
