Looking for a small size low-noise 16-channel ADC card

Hi

I need a small size (approx 50mm by 100mm) low noise 16-channel ADC card sending data on gigabit ethernet for real time data acquistion.
Minimum sampling (simultaneous) requirement per channel is approx 48 Ksamp/s/ch.

I know i ask for lot of things in very small size but if any one know some commercially available adc cards near these specification, let me know it please.
