Load Regulation - voltage drops significantily

Load Regulation

I was trying to put some load on my SMPS design (on the 5V rail), but the voltage drop significantily. I don't know what can I do to fix it

1. Does cutting out the solder mask on the DC side helps?
2. I only add heavy loading to the 5V rail but not other rails, could that be the problem?
3. Maybe the grounding not so good?
4. Or anything else? also the ripple is a bit high too

Please help!
