ioctl fails to access the firmware

After loading the firmware, the app start sending the MIPs to the firmware.
The error messages shown as below were got:

MIPs microcode FAILED to download!
Firmware runchip_ioctl(0): [_ALLOC_CMD_BLOCK] (0) virtAddr=0xc1b58000 busAddr=0x01b58000

Anyone could give me some suggestions or advices on this problem?
 

The issue can be traced now...

There are 2 possibilities:
1)memory mapping wrong;
2)ioctl instrunction wrong.
