Look in beyondlogic.org and allowio it seem to be simple and usefull
 

So, antoher time, there is DlPortio and or UserPort that do the job for you !

for example :

(DlPortIo offer function that can be use to acess IoPort, UserPort permit ALL program to simply use inp(), inportb(), outportb(), and so on, to all process)
 

I posted BeyondLogic prog in the topic about XP access (same Programming forum)

It should work under NT server.

Windows 95/NT Port I/O Driver
 

The easiest way is to use GIVEIO driver.
It allows to use standard C functions
inpb inpw inpd outpb oupw outpd.

Try to find GIVEIO with any search eng.
