How to find out solution of state model equations?

I have some state model equations namely:
x'(t)=Ax(t)+Bu(t) and
y(t)=Cx(t)+D u(t)

A,B,C,D are the matrices state transition matrix, input matrix,output matrix and detailed output matrix respectively.

I know how to find out these matrices,but my problem is in finding value of x' and y.
I dont know how to solve these both equations?

If anyone have information regarding this then please let me know.
