how to calculate the equivalent input noise for an op?

if we can calculate the equivalent input noise by integrating the input noise spectrum density? if this is true, to which point should we integrate?
 

plz refer to Razavi's book, "Design of Analog CMOS Integrated Circuits", chapter 7 and 9.

I have another question, there are two kinds of specs for a general op, vp-p and voltage spectrum density, why some datasheets give vp-p as well as voltage spectrum density, moreover, some datasheets with vp-p from 0.1Hz to 10Hz and that some from 0.1Hz to 1MHz?
