How kernel know how memory and I/O are mapped?

How Operating system kernel know how and where physical memory and I/O devices like VGA PCI and USB host interface are connected to cpu ? How a kernel developer knows everything that ?

There will be different kernels for different mother boards ? How the discipline is followed.
