hardware design of USB transceiver ( physical layer)

I was wondering what is the design of USB transceiver (for example USB 1.1)?
I know it us some kind of differential transceiver, but what is the actual physical hardware design and what is done after the differential D+ D- added?

The required functionality is described in USB spec chapter 7. Everything else is up to you.
