FT232 circuit not working

Dear friends,
I need a dual USB to serial converter. for that i am using two FT232 chip with mini USB connector & a common MAX3232 chip for both channel.
I have attached my schematic diagram & PCB layout. i can not able to instal the driver. please anybody help me....:cry:

What operating system... Windows? Does the system alert you when you plug in the USB, that it has found a new device? Is there any activity from the PC when you plug it in?
 

windows 7 ultimate - 64bit... I think my attached screenshot will be useful to solve the issue... I don't have problems in my PCB (short ckt, track missing, etc...,). My chip is also from digikey, so i hope its original part...
Is there any problem with my circuit diagram? Now my VCCIO also USB powered(5V). I can measure the USB supply & its 5.1V...
I am using mini USB & the connections are OK & the pin diagram is shown in the schematic diagram.... Please need help to solve this issue...

The PCB connection are as per schematic... if anybody need i can post the Gerber also....
Meanwhile today i am going to install it in a windows XP & i will comeback with the result....

Just a quick note (I'm on the way out)...

The first image showing "Usb device malfunctioned" error usually means that there is a problem with the device, nothing to do with the driver at that stage. I've only seen that error when either the device is actually faulty (could be hardware or misconfiguration of USB mode), or on one occasion when I was using the device with software in a VMWare virtual machine and continually swapping the device between the host and VM for testing.

Thank you very much for your reply. I will recheck my PCB tracks again. First I am having doubt with my Schematic diagram & USB connection. If you have any chance to analyze my schematic diagram, it will be very useful to me. Thank you again....
