Excel sheet for finding risk of failure

Hi all,
does any one having idea of Spread sheet-Excel....

for finding the risk of failure of the Electronics design/product......

can any one suggest, what factors can contridute to make this type of sheet...

have any one used a Excel file....for this purpose....

can I have a sample copy...pls
