Equation of a Plane with rectangular base

Dear all,

Please, can anyone help me out? i need to find the equation that describes a plane. The plane can be viewed from the bottom as a rectangle and viewed from top as a '' rectangular table top'' with 4 different height of legs. I'm in need of the equation that describes the top view (plane).

If the plane is in a three-dimensional world, you'll find that three (not four) leg heights describe it.
Is yours in a four-dimensional contex? If not, mathworld.wolfram.com/Plane.html will serve as a good starting point.

Thanks. It's actually three-dimensional but with four heights describing it.
