- "Transmission Lines for Digital and Communication Networks : An Introduction to Transmission Lines, High-Frequency and High-Speed Pulse characteristics" by Richard E. Matick, Tom Knight

This one is classic, well worth reading.

Another excellent book


-"High Speed Digital Design" by Howard Johnson

Famous "Black Magic hanbook" has a chapter on transmission lines and another one on related topic of terminations.

i used to use supercompac (i think thats what it was called) run under unix at work,
it was great for transmission line synthesis.

As calculator you might use LineCalc from Agilent (included in ADS), for a first rough idea, then optimize your project with an EM simulator ( i. e. Momentum ).
