Vertical MOSFET, e.g. all power MOSFET have the substrate tied to source by design. Respectively there's always a "body diode" connected in parallel to DS.
Small signal MOSFET have the source terminal junction-isolated from substrate, but many discrete devices have source and substrate internally connected though. A few discrete devices are equipped with separate substrate terminal.
