the design procedure is simple.Just derive the kp and kn and lambda values from simulation.Then do your hand calculations ,and then go for design. i would recommend you refer to maloberti.

maloberti is the name of the author of cmos analog vlsi design(i dont quite remember name of the book).U can search for this author in this forum.Yes u can design for differential output in a folded cascode. Please refer to this book for the same.
