rail to rail means

rail to rail means the OP output range can extend to the supply voltage maxium dynamic range.

o/p range is upto the supply voltage and input CMIR is also upto the supply voltage.o/p rail to rail can be obtained with the help of class AB o/p stage of opamp.input rail to rail can be obtained with the help of complimentary folded cascode opamp.

rail ro rail means your input/output signal can swing from Vdd to Vss and your op amp still can function correctly.
rail ro rail makes your op amp more power efficient.

rail to rail input : means that the circuit input require variation from the most +ve supply to the most -ve supply to function properly
actually this is not easy because it will cause the input transistor to pass through many regions of operations and in this cases we can't assume small signal analysis

rail to rail output : means the the output of a certain circuit is varying from the most +ve supply to the most -ve supply.

actually : If you aim to design a circuit with input or output or both varying rail to rail, it will require deep understanding for many parameters of the design you have

Rail-to-Rail means (as the others mention) from the max available voltage to the min voltage. I post a reply to another topic "rail to rail opamp". there I suggest a paper by Ron Hogervorst about circuit techniques for rail-to-rail input and output techniques. here it is :
"A Compact Power-Efficient 3 V CMOS Rail-to-Rail Input/Output Operational Amplifier for VLSI Cell Libraries" , Ron Hogervorst et al , IEEE JSSC 1994
