I have a "computer control survey", by Wittenmark.
Contains one chapter (ch8) on pid design with principles, schematics,
practical aspects and an example of code written in c.
Very good for beginner.

Also, Microchip has an AppNote about PID motor control with PIC16F877 and 18F45x. In the appnote, the are schematics, source code in C etc.
