You design an antenna to radiate on purpose. Usually you don't want to do this with transmission lines. There are some ways to avoid radiation, like confining the wave to a medium such as a coaxial cable or a waveguide. In a microstrip usually you don't want radiation, but if you design it carelessly you may end up with an unwanted antenna...
